TEST TUBE BABY

WHAT IS A TEST TUBE BABY?
Insemination of the stimulated ovaries of the woman’s eggs with the help of medication, gathered by ultrasonography in a laboratory environment and obtaining pregnancy by placing the developed embryos into the uterus. The response differs from person to person and different responses can be obtained from each test tube baby assays.


The treatment has five main steps:
• The completion of the guidance and examination of the couples (hormone tests on the 2nd or 32nd day of the menstruation period, vaginal ultrasonography, sperm test of the man and hysterosalfingraphy/ office hysteroscopy in order to evaluate the uterus)
• Pressuring of the ovaries with medication (the purpose of this procedure is to prevent the eggs from breaking and thus falling into the stomach and to obtain more embryos.)
• Growing and maturing the eggs with the help of medicine (the purpose of this procedure is to obtain many candidate eggs that will develop embryos.)
• Gathering of the eggs through the vagina (bulp) under surgical conditions with the assistance of ultrasonography.
• The automatic fertilization of the eggs with the sperm (IVF) or the injection of a single egg with a single sperm (ICSI)
• Placement of the developing embryos into the mothers womb.


WHAT IS THE SUCCESS RATE OF THE TEST TUBE BABY TREATMENT?
Pregnancy rates differ from center to center and the accepted pregnancy rate in the world is 50-55% for getting pregnant and 35% for taking the baby home. The rest of the 30% still cannot conceive despite the modern medical developments. The success rate of the test tube baby treatment is higher with young and with no previous test tube baby experience couples. If the woman is over 35 years old the success rate is lower.

HOW DO THE EGGS DEVELOP DURING A TEST TUBE BABY TREATMENT?

On the 2nd-3rd day of the menstruation period the medicine called gonadotropin, which is known to cause the enlargement of the eggs, is given. During the medical treatment the enlargement, the eggs are followed through ultrasonography and estradiol hormone level in the blood is monitored and when the target egg reaches the desired size (at lest 3 eggs of 17-18cm) in order to provide the last matureness, the cracking injection (HCG) 5000-10000 IU is performed.

HOW ARE THE EGGS GATHERED?

The gathering procedure starts after 32-36 hours following the cracking injection. Egg gathering procedure is carried out in the surgery room of our clinic together with vaginal ultrasonography under anesthesia and the eggs are then immediately sent to the laboratory. The procedure of gathering eggs from both of the ovaries will take approximately 15-30 minutes.

Sometimes even though there are many eggs no egg cellules can be obtained. The most important reason for this condition is that the cracking injection is made at the wrong time. In order to obtain good quality eggs it is very important to do the cracking injection at the right time and to be present at the clinic at the specified time for the gathering of the eggs.

WHAT TYPES OF PROCEDURES ARE EXECUTED TO THE GATHERED EGGS?

First of all the gathered eggs are cleaned, then placed into a culture liquid and deposited in incubators which are chambers with a specific temperature. The incubator keeps the heat at 37o C and stabilizes the carbon dioxide level at 5 – 6%.

Mature egg cellules then become ready for insemination after 4-6 hours. After stimulation 80% of the eggs with 18 - 22 mm diameter are considered as mature eggs suitable for insemination. The following procedures mad differ according to test tube baby or microinjection.

HOW ARE THE SPERMS OF THE SPOUSE OBTAINED?

On the day of the egg gathering your spouse is called to the clinic to deliver his sperm. Before gathering the eggs a sexual diet of 3 to 5 days is necessary. Sperm is delivered by your spouse through masturbation in specially prepared sperm delivery room and first liquefaction is anticipated. Liquefied semen is examined by sperm count, activity and shape, after undergoing through several processes in the laboratory the best quality sperms are chosen.

For patients who cannot deliver sperm by masturbation or for men with no sperm in their semen an operation consisting of taking many pieces from the testicles in order to collect sperm is carried out. Wit this method called TESE, depending on the reasons, the possibility of finding sperm differs between 25-60%. This procedure is executed under local anesthesia.

WHAT IS A TEST TUBE BABY (IVF) PROCEDURE?

50.000-100.000 sperm is placed in the same cup with each egg. In the case of defected sperm parameters this number can be increased. Waiting period for the sperm to enter and inseminate the egg begins. Inseminated egg cellules divide and develop the embryos. Embryos are controlled every day and are generated to grow 2-3 days in the laboratory environment.

IVF is generally a preferred treatment for patients who cannot get pregnant due to occlusions in the tubes.
